Apply Formatting Specifications to node-lists
You learned that formatting specifications are applied to SGML-elements.
But actual first the grove is constructed from the SGML-document.
And then the formatting specifications are applied to the grove.
Before the formatting specifications are applied to the nodes the
grove can be altered by the DSSSL-stylesheet.
E.g. you can generate an extra branch which contains a table-of-contents.
